Some floaters need urgent care.

Most floaters are harmless. But new or sudden floaters can be a sign of a serious problem at the back of the eye, such as a retinal tear or detachment, which can lead to permanent sight loss if it isn’t treated. ¹⁵˒¹⁶

UK: ask an optician for an urgent appointment or call NHS 111; if you can’t be seen urgently, go to a hospital eye casualty department or A&E. US: see an eye doctor or go to the emergency room right away.

If you notice any of these, get urgent eye care, the same day if you can:

Floaters or flashes for the first time

A sudden shower of new floaters, or a sudden increase in floaters or flashes

Flashes of light

A dark curtain or shadow across your vision

Blurred vision, sudden vision loss or eye pain

Floaters after eye surgery or an injury

Clearer® is for long-standing floaters that have been checked, not for new symptoms.

How floaters form

A shadow, not a surface mark.

Floaters aren’t on your eye’s surface. As the gel inside your eye breaks down, its collagen fibres clump into strands that drift as your eyes move. You see their shadow on the retina.

Inside the vitreous

Inside your eye, the gel is coming apart.

Why it changes

The breakdown has more than one cause.

It’s cumulative, and no single factor explains every case. Age, sex, general health and short sight all play a part; nutrition research targets oxidative stress and glycation.

An older man looking up at a clear blue sky.

Natural ageing

A coating keeps the gel’s collagen fibres apart. In donor eyes it halved about every 11 years, leaving fibres to clump into strands. ¹

Sunglasses on a white sea wall in hard sunlight.

Oxidative stress

Light entering the eye creates reactive molecules that can break down hyaluronan, the gel’s water-holder. Antioxidant defences in the gel tend to weaken with age. ²

Illustration of collagen fibres with a few amber cross-links between them.

Glycation

In everyone, sugars slowly bond to the gel’s collagen, cross-linking the fibres and changing how they hold together. High blood sugar speeds it up. ⁴˒⁵˒⁶

Also linked with earlier change

Short sight (myopia)

In longer, short-sighted eyes the gel tends to liquefy and detach earlier, so floaters can start younger. ⁷˒⁸

Menopause and hormones

From 60, women’s gel tends to detach sooner than men’s; one study tied it to menopause, possibly through falling oestrogen. ⁹˒¹⁰

Metabolic and eye health

Some studies link diabetes to earlier gel ageing; eye inflammation, injury and cataract surgery are linked with earlier change. ⁶˒⁷˒¹¹˒¹²

Factors linked in research, not a diagnosis of your eyes.

What age changes

Age changes the material itself.

On average, the gel is a quarter liquid at 40, a third at 60 and about half by the late 80s.

Between waiting it out and surgery.

Often advised

Wait it out and learn to adapt

Many people learn to tune floaters out, but not everyone, and the gel keeps changing with age.

New approach

Proactive care for your eyes

Shield your eyes from strong sun and UV, and eat leafy greens for lutein, which is concentrated in the macula.

New research shows targeted nutrients can help: in a 2021 placebo-controlled trial, they eased floater discomfort. ¹⁴˒²⁶

For severe cases

Laser treatment or eye surgery

Laser or vitrectomy surgery may be offered after specialist assessment. Both carry risks.

The research question

Can targeted nutrition ease floater discomfort?

If oxidative stress and glycation help break down the gel, antioxidant and anti-glycation nutrients are worth testing. A placebo-controlled trial in Ireland, published in 2021, did.

The FLIES trial (Floater Intervention Study)

On the formula, floaters bothered people less and opacity area fell.

61 adults · six months · double-blind · placebo-controlled

Primary outcome

Floater discomfort

3.90 to 2.10

Average floater discomfort on the study formula (0–6 scale) ¹⁴

Average score on the study’s 0–6 floater discomfort scale, at the start and after six months.

Placebo: 3.69 to 3.35, not a significant change

11 of 30 on the formula said their floaters were stable and no longer bothered them, versus 4 of 26 on placebo.

People on the formula also reported floaters affecting daily life less (1.37 to 0.73 on a 0–4 scale); placebo barely changed (1.08 to 1.00).

Imaging outcome

Vitreous opacity area

20 of 26

people on the formula had a decrease in measured vitreous opacity area ¹⁴

Ultra-widefield infrared scan of an eye with floaters: on the left the opacities show as dark shadows on the retina; on the right the same scan with each shadow outlined, as when opacity area is measured.

Example infrared scan, not from FLIES. Adapted from García-Aguirre et al., J Clin Med 2022, CC BY 4.0.

Placebo: 6 of 21

Measured on a 102° ultra-widefield infrared video of the vitreous: a masked investigator traced the outline of every opacity, the clumps that cast floater shadows, on five still images per visit.

Average opacity area fell significantly on the formula; on placebo it didn’t change significantly.

Sharpness on the eye chart didn’t change in either group. One of several contrast tests (how well you see low-contrast detail) improved slightly with the formula (0.70 to 0.76 logCS, P = 0.047); placebo didn’t change. The authors suggest this reflects less light scatter from the opacities.

What to expect

What the first six months look like.

Clearer® is a daily routine that builds over time. The FLIES study measured results at six months, so that’s the point to judge it.

  1. Month 1Build the habitOne capsule a day with a meal; a bottle lasts a month. Note your floaters now, to compare later.
  2. Months 2–3Keep it goingSame time, same meal. Some customers notice floaters changing by now; others don’t. Both are normal.
  3. Months 4–6The research windowSix months is when FLIES measured floater discomfort and vitreous opacity area. Compare with your month-one notes.
  4. Beyond six monthsLong-term careThe gel keeps ageing, so eye care is long term. Keep going alongside regular eye checks; sudden changes need urgent care.

Questions

Straight answers before you start.

Don’t wait if floaters change suddenly

Get urgent eye care, the same day if you can, for: floaters or flashes you’re seeing for the first time; a sudden shower of new floaters, or a sudden increase in floaters or flashes; flashes of light; a dark curtain or shadow across your vision; blurred vision, sudden loss of vision or eye pain; or floaters after eye surgery or an injury. These can signal a retinal tear or detachment. UK: ask an optician for an urgent appointment or call NHS 111; if you can’t be seen urgently, go to a hospital eye casualty department or A&E. US: see an eye doctor or go to the emergency room right away. ¹⁵˒¹⁶
